"MAKE A POINT WITH SAFETY" RULES

Who is eligible to participate:
All ABC Company Employees

How long will the program run:
January 1, 2003 - December 31, 2003

Individual Awards:
Each month you work without a Lost Time accident, you will earn 2,000 award points.  Qualifier: If you have a Lost Time accident, you will forfeit any points for that month. (All previous award points remain valid.  This only affects you for the month the accident occurred.)

Team Awards:
Each month that your team works without a Lost Time accident, all employees of your team will earn an additional 2,000 bonus points.  Qualifier: If a member of your team has a Lost Time accident, your entire team will forfeit your team points for that month. (All previous award points remain valid.  This only affects your team bonus for the month the accident occurred.)

Education Awards:
Each month you will be eligible to participate in a safety quiz that tests your knowledge on the company's safety policy and procedures.  All you have to do to participate is dial the Automated Telephone Award Center at 1-800-743-5136 and listen to the easy to follow prompts.  If you answer all the questions correctly, you will be awarded 1,000 points directly to your account.

Each month you will receive an activity statement detailing the points you earned during the previous month, as well as your total program earnings.  You can save or spend your points as you wish during the program.  If you like, you can break your points up to get multiple awards. (Points have no cash value.)

When you have acquired a sufficient number of award points to obtain the awards(s) of your choice you have two options: ONE: Completely fill in the order form section of your activity statement, including the number of points required for each selection, and submit it to: ABC COMPANY - GIFT HEADQUARTERS, P.O. Box 1234, Maintown, USA 56789 (or) TWO: Call the Automated Telephone Award Center at 800-743-5136 and key in your selection(s).

All awards will be shipped directly to the address (no P.O. boxes) you specify with postage and/or shipping charges prepaid.  In the event a parcel arrives in a damaged or unsatisfactory condition, please notify the Participant Services Department at the above address or at 1-800-555-5555.  Please do not return awards without specific instructions.

Please allow 2 to 5 weeks for delivery.  If, in the rare instance, your selection cannot be delivered immediately, you will be notified by postcard.  If your selection becomes unavailable, it may be substituted with one of equal or greater value.  Shipments cannot be made outside the continental U.S.
